The deadline to apply for U.S. Service Academy nominations with our office is October 15, 2025. Please mail completed application to my Costa Mesa District office or drop off completed applications in person at 1370 Adams Ave Suite A Costa Mesa, CA 92626.

Once you fill out the form below, it will generate a PDF which will be a component of your completed physical packet. The PDF will include information regarding other application materials you will need for a completed packet.
