The deadline to apply for U.S. Service Academy nominations with our office is October 14, 2026. Please mail completed application to my Costa Mesa District office or drop off completed applications in person at 1370 Adams Ave Suite A Costa Mesa, CA 92626. Our office is open Monday – Friday from 9 AM—5 PM.

Once you complete the form below, a PDF will be generated for your physical application packet. The PDF will include the additional required materials and instructions for submitting your completed packet.
